[ ] find a way to include line number of every maker() call

Remove 'markup' from the following IDE commands:

[x] (ctmh), (ctms) -> (cth), (cts)
[x] (fnmh), (fnms) -> (fnh), (fns)
[x] (lmimh), (lmims) -> (lmih), (lmis)
[x] (lmnmh), (lmnms) -> (lmnh), (lmns)
[x] (mnmh), (mnms) -> (mnh), (mns)
[x] (spmh), (spms) -> (sph), (sps)
[x] (snmh), (snms) -> (snh), (sns)

Local measure index, local measure number, measure number, stage number all embed annotation spanners.

[ ] dynamically adjust Y positions to allow multiple annotation spanners to visible at the same time.
