Fix division by zero and index-out-of-bounds bugs #9

Fixed a couple of bugs that cause exceptions when user presently does not "own" options, but would still like to generate reports. Update .gitignore to prevent pushing personal data. Unsure if "symbol_and_instrument_urls" should be under version control or not, but will investigate. I did not push update copy left one include in the repository.
