Add GitHub issue forms #222

Following PR improves the Create issue form in github.

nit: commit and PR should probably say Add form based GitHub issue flow/Add GitHub issue forms. 😄

Is there some way to "demo" these before I press merge? Or should I merge this into a test repo of mine and check?
