Make use of GitHub Forms #9898

I just learned that "GitHub Forms" is a thing, which basically allows us to enhance our current issue markdown templates with actual text fields. I just randomly stumbled upon such a form and it looks so good.

It's basically an evolution from the markdown "template" we're using to an actual form
