Improvements for the "Create component" dialog #5

As you can see in the screenshot, there are two issues:

- THe title "Add a Tapestry 5 page" is wrong in a "Create component" dialog

- In Tapestry it is possible to create components without templates. Please 
provide an additional checkbox, labeled with "Create template" which is checked 
by default.
