Add Appendix B & formatting fixes

Added the full text for Appendix B and also these formatting fixes:

Made all heading levels uniform (one hash for top level, two for second level, etc.)
Made all figure captions bold
Corrected figure numbering to account for Figure 4 being removed
Standardized capitalization in headings
