This PR makes some additional changes to the Gazetter to standardize and reduce deeply nested folders.
Standardization
Standardize Rivers to use the naming of just rather than River , i.e. Istaros instead of River Istaros in the file name.
Eliminate "generic" place names in favor of specific ones (i.e. Ogre Camp (Cleenseau) instead of Orgre Camp, or Dwarven Outpost (Raven's Hold) instead of Dwarven Outpost)
Remove "The" from all place names except inns and similar buildings (where The is part of the name) and regions that don't have a better identity
Moved all historical locations to History
Reduce Nesting
I reduced nesting somewhat in the Western Green Sea Region by getting rid of Greater Sembara and promoting the various subpieces, and I reduced the high number of top level files by adding an few sections.
