Append Signature Page DCTM Implementation Error Handling #5

The TSGDctmAppendSignaturePageImpl doesn't appear to handle gracefully the following three cases:

No PDF rendition
PDF rendition, but primary rendition
PDF as secondary rendition

Need to ensure that all three situations work, and also need to make a decision on what should happen for case 1 (no rendition exists)

one way to handle case 1 - In AW 4.0, if a signature was required on the workflow docs for one of the tripped roles, the wizard wouldn't let you even start the approval route.
