Facility registration!! #181

(from google docs meeting 7/23) New Facility Registration: Add “Facility Administrator/CI Coordinator” field. Explain clearly that this field will mark the person who is the central administrator for their facility. “Services provided at your facility (select all that apply)”: Audiology, Surgery, Speech Therapy, Other “Please list the approximate population of the city in which you practice”: (Appropriate number ranges for populations) If surgery is picked - “Please list the level of Cochlear training completed by the surgeons”: General Otolaryngologist, Fellowship Trained Otologist “Would you describe your facility as (select all that apply)”: Academic, Tertiary, Community-based, Private practice

@drjed Besides the security assessment and the payment portion, is there anything else to be done on the facility registration?

I don't think so. Otherwise I like it
