Record book, section, and subsection access #118

(per request from Ryan) App should emit an xapi statement whenever a book, section, or subsection is loaded.

This works at the book level as of c1bf7d7749be828ab5a581f6a29cce3e69d02551, but not for *sections.

Once #121 is merged, this issue will be 2/3 complete. We will generate xapi logs for opening books and sections, but not subsections.
