Update prerequisites to latest published versions (notably to pull in a VS2019-compatible OutputWindow implementation);
Update upper ranges of compatible VS versions to include VS2019;
Add link to eula.rtf to allow Chapel code to compile;
Add small VSIX project icon to Chapel manifest to match other manifests.

Tested on my own VS2019 Community on a live project; for what I use it for, it's working fine.
