Terminology Data Dictionaries Issues #284

Several of the terminology sets are missing data dictionaries. For ex: admit source and admit type are both missing data dictionaries.

Every terminology set should have a data dictionary. Review every terminology set and if the data dictionary is missing or out of date, fix it.
