Update Data Marts Section: Readmissions #313

There should be 4 sub-sections called: "Overview", "Instructions", "Data Dictionary", "Analytics". The "Overview" sub-section should have at least a simple overview of what this data mart is about. The "Instructions" sub-section should have some basic instructions related to this data mart. The "Analytics" sub-section needs to have at least one example of a query that uses each output table in the data mart.
