Some feedback on this PR...

• Pretty sure data_source HAS TO be part of the PK.
• Need to explain the difference between patient_id and member_id, otherwise I have no idea how to map the data
• Need to explain the difference between payer and plan, otherwise I have no idea how to map the data
• admit source and type aren't really used for any analytics and therefore we should remove from audit
• the audit should check the following fields for valid values but the explanation doesn't say that it does...

Instead the instructions just say something really generic like "The mapping audit checks that the value of this field is a three-character string and that it is consistent across all lines for a given claim_id."
