CMS HCC Feature request: Ability to break down disease risk factors by severity #175

Thanks @Aaron and @Sarah. Super helpful!! Adding a "factor_type" field would be super helpful! Also, breaking down the "Disease" section by chronic vs acute would also be super helpful. This would be a clinical judgement call but I'm happy to provide a starting point. I think the areas of disagreement would be at the margins.
