Mapping issue for with code type

Describe the bug The terminology__code_type.csv seed file contains two entries for hcpcs codes; however, most our connectors and mapping logic map codes to a one code type hcpcs. We need to decide on a standard code type and update the terminology or connectors/mapping.

code_type icd-9-cm icd-10-cm icd-9-pcs icd-10-pcs hcpcs level 1 hcpcs level 2
