BUG FIX: CMS Chronic Conditions staging references

This data mart was referencing the Input Layer medical and pharmacy models rather than Core. This was causing issues in new marts when trying to just seed terminology and not run any data marts.

How has this been tested?

Ran dbt build --full-refresh with no errors.
