MedicalAudience ... let's check that Audience navigation looks ok if we break out audiences this way; otherwise core should be fine. (See current proposal)

seems to reach a conclusion that MedicalAudience remains in extension. also follow https://github.com/schemaorg/schemaorg/issues/492
