Add X-Accel-Buffering header #2

@linonetwo I'm adding this in a separate release. I've added these changes myself since there are conflicts so I'm just going to close this. This will be in version 1.0.3 so you can use 1.0.2 if you experience issues with it. Please let me know if anyone reports bugs with this. I could add it as a configuration tiddler so the headers can be customized but I'll just hope this works for now.
