Added a section for reporting issues with commit c96cbb2f3cc2329a6a703839a0800d80b9c84519

Maybe contact project core committers (listed in the "authors" section of readme.md), or @twitteross and/or email to opensource+codeofconduct@twitter.com .

This both lets individual project leads/authors deal with harassment (reduce workload on you, though of course we hope the need for this would be fairly rare) and spreads awareness of conduct as being something we are all responsible for, not just @caniszczyk personally.
