Semmed integration #5

Hi Tomas,

I've extended biograkn-covid schema adding some SemMed concepts. I've also created a SemMed migrator and added relevant SemMed data in csv format.

Could you please review and test changes?

This looks really great @konradmy! I tested the loader on my machine and it works.

what's only missing is to add in the migrator.py file the migrate_semmed method @konradmy, if you could add that I will then merge your PR
