It lost the indentions with the copy and pasting, but I couldn't attach the file here since it doesn't support XML. I can upload it to the repository if that'll be better.
Indenting doesn't matter, but it also lost a bunch of angle brackets, which makes it a hassle. Either upload or just email it to me.

It looks fine to me? I am not getting an error in Oxygen at all. So weird. Can you try copying/pasting the error? You should be able mouse-over it, then hit F2 to focus, then copy/paste the text. That might help.
So, I get an error when I try to open it via the File Exchange feature on CourseWeb. Chris got a similar error with his, and it was fine when I just took out that line. I'll forward you what he sent me via e-mail.
I wonder if that File Exchange is messing with the XML? Trying to render it or something? The file you uploaded here is fine. Maybe share via email or box instead? Who knows what courseweb is doing with those files.
In theory is could be the quote symbol in your extent. That is technically a reserved character, though it typically doesn't cause issues. You could try replacing that with 'inches', which is probably better for you metadata anyway, as " is kind of a short hand and may not be recognizable by all users.
I tried it with inches yesterday, and that didn't fix it, but I will change it to inches per your other point. But when I take out the form subelement, it works just fine.

Just to clarify, are you seeing this error message in Oxygen, or only in the File Exchange tool? The record is fine as far as MODS/XML is concerned, as far as I can tell. I really think File Exchange is corrupting something, to be honest.
Just the File Exchange tool! It's really weird because the issue seems inexplicable.
I'll let Chris know. He tried removing the offending line, which was valid in Oxygen, but then it just threw up another error.

For some reason, the following line is causing issues, and I can't figure it out. Help please! π
