UI Improvements to Add Event #561

When setting up an evaluation, the options you get to pick should depend on which evaluation type is selected. For example, when setting up a mixed evaluation, we should not be able to set the "Comments Required" flag - it does not apply to mixed evaluations.

There should also be clear indication of which fields are mandatory.
