Copied README to index.md for readthedocs #254

I've imported the project to read the docs but the docs need an index.md to populate the main index page.

Thanks for doing this!

Would be nice if either readthedocs or github can read from a configurable location, just so we dont need to keep two files around. If not, could you add a Makefile target to re-generate index.md when readme.md is changed?
