The documentation sample for \fv, at https://ubsicap.github.io/usfm/notes_basic/fnotes.html#fv-fv uses "plain" ("non-plussed" / nested) \fv.

In the context of the sample, and indeed in most contexts, +fv not \fv should be used. This is strongly IMPLIED by the note:
This marker will typically be nested within another footnote content element, like \ft, \fq or \fqa. See Nesting Character Markup for details.
The sample contradicts this by using plain (non-plussed) \fv.
