Study Bible stylesheet table headers only support 8 columns #139

The tables for study bibles support 12 columns: tc12, thc12, tcc12, etc). The table header only supports up to 8 columns "th8". I will create a pull request to update usfm_sb.sty to support th9-th12, too.
