Use QR code for patient registration #206

Having added QR codes for multi-factor-authentication, I wonder if we could use them for patient registration on the app. We could display the code in the body of the email, on a secret link from the server or on a piece of paper sent to the patient.

It would be quite a bit of work to implement so maybe wait to see how much difficulty patients are having registering using the existing methods.
