Goal

Improve the styling of the forum creation page

Suggested Edits

Here is the current page:

• [x] Header text that says "Create new forum"
• [x] Fields with the text box beneath it
• [ ] Radio buttons for the "Forum type" field (see issue #379)
• [x] Change "Save" to "Create"
• [x] Add styling to "Create" button
• [x] Shrink the size of the "Description" field

Reason for reopening

Since issue #379 hasn't yet been closed, I'm going to reopen this issue and move it to the backlog.
