Updates to Catalog Report a Record Problem form #226

We need to update http://forms2.lib.uchicago.edu/lib/problemreport/problemreport.php The is the Report a Record Problem form reached from a VuFind full record page, such as https://catalog.lib.uchicago.edu/vufind/Record/5925760 .

The edits for #1 are a) make email a required field. b) Add a field directly after email for Affiliation (with the following options) o University of Chicago College Student o University of Chicago Graduate Student o University of Chicago Faculty o University of Chicago Staff o University of Chicago Alumnus/a o Not Affiliated with the University of Chicago

c) Add the "This form is powered by Springshare and complies with its privacy policy." note to the footer. d) Consult with Keith about the new form target.

@dbietila - for task C: is this form actually powered by Springshare? I thought we only needed to add this to the LibAnswers forms?
