Fix the tab order issue on Submit an App and Edit App pages #216

@krozett On both Submit an App and Edit App pages, the tab order skips the expanded form “Authors” textboxes and “Remove” button after activating the “Add Author” button using the keyboard. Same issue when activating the “Add Organization” button.

Same tab order issue on Edit App pages, when activating the "add LTI configuration" button.
