Removes Kaltura integration. #1885

This removes all references to Kaltura from our application code. It resets the browse-everything gem to the Samvera repository at version 0.14.2.

Coverage remained the same at 96.17% when pulling c5df3a5c6169926c325478f41db5488655b495d2 on remove/Kaltura_scholar into 0ec6c4b17854e72ac38abdb9e80e6e2a6e854f2b on develop.

@scherztc You should also edit https://github.com/uclibs/scholar_uc/blob/develop/config/locales/hyrax.en.yml#L65 to remove "Dropbox, Google Drive, or Kaltura." At the moment that text still displays in the green box on the Files tab.
