Conservators note / Abbreviated treatment report data missing #417

Migration from Access db did not include "Conservator's note" data, which should map to field "Abbreviated Treatment Report."

  These items should have abbreviated treatment reports.  They are present in the Access Database (pulled from the field Conservator’s note):
