Weird Feedback Form on ITS #306

ITS feedback form looks totally different from the others. Not sure if this is site specific. See: http://its-mobile.ucsc.edu/feedback.php image

I'm confused by this one...

  1. the global left nav block is visible in the system, but it isn't there when the page is published.
  2. the submit button is missing. It's not even in the HTML.
  3. The sidebar div for the left column isn't in the HTML either.

I've double-checked the region assignments for the template and they are correct. I've checked the publish log for errors and there aren't any.

Assigning to Tom to see if he can come up with any ideas.
