no `Data Dictionary` tab #112

README in section 1.d (also note inconsistent numbering in README) mentions a Data Dictionary tab, but no such tab exists.

I love bugs in the documentation--they're the easiest to fix!

I've renamed the "field descriptions" tab to "Data Dictionary" so the readme is now correct :) Re inconsistent numbering: I double-checked the numbering, and I don't think it is inconsistent ... I think it just looks that way because there are multiple levels (e.g., 1.a.ii) and they aren't indented. I added indenting.
