Canonical form not described #133

Appendix D says "The canonical form is as described in Section 2.13.1.3." However that section doesn't exist. Also, searching for the word "canonical" doesn't bring up anything inside the document.

OK, this was cut and paste from some other document, and it's not even quite clear which one nor where to find it. So I removed the broken reference.
