Bump ubuntu server version to 14.04.5 #49

The default ubuntu subversion of 14.04 is now 14.04.5 with a different sha256 checksum value and different vagrant box command for the result image for consistency.

This PR doesn't update the second PACKER_BOX_NAME, so I am deferring to the older #28
