Add a functionality feature to restrict a doctor to add visit notes before continue add provisional diagnosis

Problem Currently a doctor may skip writing patient visit notes history and continue with only medication or adding provisional diagnosis and confirmed diagnosis. It brings challenges whenever another clinician wants to trace history of treatment for patient, we have realize that most of doctor s skip writing clinicals notes for a patient.
