Remove unnecessary summaries #16

Review the script output and remove any summaries that do not get used for the analysis. Candidates:

Risk Analysis: more common formats to use for manual risk analysis (will do automatically for all now)
Type by Group; Name by Group; Type by Name (are simpler summaries with group overlap)

Did remove just those 4, as well as the function two_categories(), which was only used for these summaries.

Leaving Format Name Ranges, Format Name Sizes, Format ID Ranges, and Forma ID Sizes for now. However, these may be replaced by histograms through Issue 19.
