Start Well Inventory

Once the Site information and location are saved, the app kicks to the well inventory page (currently /site/3/add-well). This is also the first page the user sees when they add a new inventory to an existing site.

Fields on this page:

• Site Name (populated with the site name entered on site info page) - UICFacility.FacilityName
• UIC inventory form order number - stored in db field UICAuthorization.OrderNumber
• Well Subclass Type - stored in db field UICWell.WellSubClass
  • Look up table is UICWellSubClassDomain
  • If General is chosen the db field remains empty until admin assigns the type (#58). See this explanation.

Site name should only be visible on the well creation page, and not editable.

Given the client's request to have the Site Name persist across all Site pages, what do you think about having the name on all the Well Inventory pages as well? If so, I'll create a new issue for it.
