As of right now upon sign-up the user has no way to associate themselves with a hospital or provider.

Rationale
Allowing a user to choose which hospital/provider they are affiliated with will allow more meaningful analytics.
Implementation Ideas
The sign-up page should now include a dropdown for the user to select which hospital they are affiliated with.

They may either choose one or the other just like it is now.
