Apply Bootstrap styles to index views #19

Wondering if the headings for the fields 'title', 'format' and 'date' could be left justified like the entries. Would make it easier to read, I think.

We might mark each table with the Bootstrap classes table and table-striped. This would (among other things) cause the headings to be left-justified.
