add TVB myTV support #116

網上看了某blog說 用changehttprequestheader extension把 tvb.com 整個 domain 的xforwardedfor 改成 202.126.57.189

即可連接mytv, 點播和直播都OK 只是速度不怎樣給力就是 但總也可以看

也許可以加進unblock youku 里
