Create templates for issue requests #103

Would like a way in github create a template for users to fill out before filing issues. New feature to github, see https://github.com/blog/2111-issue-and-pull-request-templates.

FWIW, here's a pretty generic CONTRIBUTING.md , modified from the one ROpenSci uses for all projects. Very different project of course, but the instructions about setting up clones for PR might help.
