Survey Assessment needs to be added after Post Assessment #23

This would be an optional part. PO should have an option to enable or disable Survey Part for a Training (See attached Screenshot). Survey Questions should also be separate category under Question Bank. Its Answers/Results should be shown separately and would not be added to existing Reports. Separate Report should be created for Survey Results.
