Go over the rest of the triggers from ContextD #13

First we had only the 23 negation triggers from the ContextD paper; now we added 495 triggers from this link in the paper.

Fixed a couple things in the json with the triggers, and confirmed that these now can be loaded into medspacy (but obviously haven't tested them all). See #11 for potential issues.
