Describe what your pull request addresses

[ ] Currently when a feedback report is submitted it always times out.
[ ] See issue for details on root cause and this approach to fix it, namely to change the dialog to a page

Test Instructions

[ ] login, click menu, fill out the form, then click close (without submitting)
[ ] return to form; verify that form is cleared
[ ] fill out form and submit
[ ] should get the success message. Click the "ok" to dismiss the Success message
[ ] page will close, returning to the main page.
