Service Form: Accessibility for persons with disabilities

Instead of "Accessibility for persons with disability and the options Yes and No as Radio Buttons" replace it with the following question with checkboxes

**Has the ability to support the following disabilities:

• autism spectrum disorder
• deaf or hard of hearing
• mental health conditions
• physical disability**

The new requested options are added but please note that the Yes and No options cannot be removed until all old content containing one or the other has been edited to no longer have 'Yes' or 'No' checked for this field. Once the content is cleaned up i can remove them from this configuration.

We can remove the content ahead of the training, where they will get an opportunity to update the field accordingly.. Setting up this task on #1352 to keep this in check
