I can see this warning/error in upgrade.log:

upgrade-post[27394]: redhat_upgrade_tool.util WARNING: failed to remove /system-upgrade-root: [Errno 16] Device or resource busy: '/system-upgrade-root/mnt'

Reproduced with: redhat-upgrade-tool-0.8.0-3.el6

RHEL-6 has reached EOL last year, and we're not planning any further releases.

After discussion with @shaded-enmity we've decided to close any outstanding issues that aren't causing known functional issues.
