This is normal behavior part of the exclusive access mode: the media keys are a
different process that is then forbidden to access the audio device.
What you're asking is then to hijack the media keys to control Audirvana.
This is not high priority for now, but this may eventually come into Audirvana.

Honestly, I find it strongly interferes with usability. Just to change the
volume or mute the sound, I have to find the program among my various windows
and manipulate a slider with my trackpad or mouse. As far as I can tell, there
isn't even a keyboard command for changing the volume - is that correct?
(So I at least would consider this a priority...)

The option to ignore media controls is not working, you can still use them. It
also interplays weirdly with itunes..

The option is only for volume control keys.
And it is made to play along with iTunes, as it is among the different Apple
media apps: the foreground, or the latest application to have in foreground
gets the media keys.
