Very few program files contain comment block headers #8

Right now very few of our program files contain comment block headers. I suggest that every one should have a header indicating that it is not copyrighted and indicating that it is part of this release. It should also have a banner briefly describing what it does.

Let me know if you would like me to do this. It would take 1-2 hours of work.

@simsong I can work on that; should I just use the contents of ctools/LICENSE.md?

I think it can be much shorter, but it also needs to say what the repo is and what the file does.
